Who
are "professional users"?

assist victims with creating petition and order forms for
court protection from domestic violence, such as
- battered women's programs
- victim assistance programs

- legal aid programs
- attorneys
Those who routinely
access information from the registry to ensure victim safety
and offender accountability

Learn who has access
to the information in the Registry, what information is available,
and how searches for information are made.
The use of Louisiana
Uniform Abuse Prevention Order forms is required for entry
into the Registry's database. LPOR has also created some courtesy
forms that can be used by the court, but are not mandatory.
View and download the entire set or individual blank LPOR
forms.
Forms Express software
program has been developed for professionals who routinely
assist with the creation of petitions and orders of protection.
The software allows the user to enter data via helpful computer
screens and print out completed forms singly or in combination
of multiple forms (i.e., petition, service information, and
accompanying orders). The software is free and available for
download.
The LPOR training
team ("LPOR Road Show") offers free training classes on the
use of the registry and the use of the interactive software.
The latest schedules and registration forms for training classes
are posted here.
To order forms or software on disks
or CD-Rom call LPOR, print out an order form to mail, or submit
your order online.
